Nosler Trophy Grade Failure
So I picked up a couple of boxes of Nosler Trophy Grade ammunition to break in the barrel of my new custom 280AI. Went to the range, set up and fired the first round, all good. Second shot was a miss fire. Inspected the firing pin/bolt and cleaned as per normal break in procedure. Fired the third round and upon ejecting the casing noticed that the primer had been blown out of the pocket. I then checked the first casing and sure enough, blown primer. I inspected the casings for other signs of stress or damage but did not notice anything.
This is a brand new custom built rifle that was chambered to the modern 280AI SAMMI specifications. It feeds and cycles beautifully with no jamming or tightness of any sort. The rifle was cleaned and left to cool between each shot. My question is, is this an ammunition failure or is it possible that there is an issue with the rifle ie; head spacing or something of that nature.

I had the same thing happen with a tight McGowan barrel on my 22-250. My reloads did not show any pressure signs but I had a handful of Federal factory loads that were way too hot and I even pierced a primer.

Sometimes a pierced primer will blow debris back into the firing pin and it can even cause damage to the pin and other parts of the bolt.

Your chamber was free from oil? Chamber should be dry or can cause over pressure signs. I use break clean and buff the chamber with a patch. How does your bolt face look?
A chronograph would let you know how over pressure you are using velocity.
If it were me, I'd pul the bullets, weigh the powder and drop the powder charge by 5%, then reseat the bullet.
Being that it is an AI, you should be able to use regular 280 cartridges and fire form them in your AI chamber.

Neither of my 280AI rifles would reach the velocity listed in the Nosler manual without showing pressure signs, so no, velocity alone is not always a reliable indicator of pressure, despite what some people believe.

My first inclination would be to check the chamber. Blowing the primer on the first round may have led to debris in the bolt, explaining the second round misfiring. I would give the chamber and bolt both a full inspection and cleaning, including a full disassembly of the bolt. I might even pull the bolt apart over a piece of clean white paper, to see if any shrapnel fell out of the assembly.

What kind of barrel did you put on your rifle? I went with a Ron smith cut rifled barrel on my 270ai and with my hand loads I run around min 270 load data, you will likely have to start hand loading and start a couple grains below min as it seems you have a "tight" barrel and work up from there, looking for pressure signs and using a chrono to look for velocity spikes that would indicate over pressure loads

This would be a good idea. A few years ago Federal Black Cloud had a mistake at the factory and would blow up shotguns. It would be worth a call. 1 800-285-3701
