1/16 lighting locks

what causes 1/16 lightning locks to break at the swivel ? Not where the breakaway is fixed but at the pivot pin that locks on the cable ?
Second one in a row Breakaway is still fixed to the stem of the lock lever But the top part that locks on the cable is History ?
