Bus accidents and the lack of seat belts

Another question for the bus drivers. Are you now required to file any incident reports? Do you have paper forms that are submitted after something happens?
In all the crazy incidents I remember, nothing ever happened. Not once were we ever asked to sign any report.
I ask because I am wondering how complete of a picture the world sees.

Yes, plus most bus lines have there own safety supervisor. We have to report any accident that involves blood (nose bleeds etc.) But if a student attacks another student with a weapon eg: pencil, pen, seat belt then it never happened. Same old same old, as with any other company. Minor crap looks good on paper to the governing powers, serious stuff well ...You know the rest of the story.
