Fishing pole with hook and line, big knife or axe, and 2 buckets to make wine from all the jungle fruit.
Drink and fish all day everyday lol, been stranded on an island doesn't seem like a bad idea

I'll only need three, as the deserted island would already be filled with the Maxim Hot 100 girls I was travelling with.
1- Satellite Phone ( dont need cell service then)
2- A GPS to tell search and rescue where to come and get me
Not necessarily and "item", but number three would be the want or initiative to get off the island given the other "Hot 100" problems I would have to attend to  .

Island
Having just been fishing in the mangrove swamps I'd say:
Water filter
Mosquito/sun proof clothes
Jungle hammock to get off the ground and away from the creepy crawlies
Mora knife
When I was out I brought my steripen and could purify water, it worked flawlessly.
I treated my clothes with permethrin and they were long sleeves/ legs so kept the sun off me in addition to the Mosquitos.
I'd build fish traps instead of fishing
I was amazed how much the biting ants hurt and would hate to sleep without a hammock. No fire required as its warm at night. And I could eventually get one started with a bow drill.
The mora knife is easy to sharpen on a rock and a good size to use easily although a parang or bill hook would be good too.
If I had a fifth item it would be a headnet for Mosquitos.
