The fax number for the National Contact Centre in Miramichi is (613) 825-0297. The email address iscfp-pcaf@rcmp-grc.gc.ca and the mailing address is on the form itself.

That is an outdated form. You will want to get the June 2022 version from the RCMP website instead.
