When I submit my renewal info every season to my insurance company, I must confirm use of a waiver and medical info document. Don't take waivers lightly! "Well Written" vs "Professionally" written by a lawyer, two different things!! Lucky for me, my Association allows me to use their waiver! The benefits of being a member of a professional association!
