The actual form itself is in the "Quick Links" box on the right side of the first page. Also read "Completion Tips" from the "Quick Page Links" column listed on the right side of the first page. Email works well and the last two times I applied I had the approved Form 6N1A back to me in under 2 weeks. Good luck with everything!
