Our local fish & game association received free booklets of permission forms as a promotion from someone trying to sell them. They just didn't work because they were too long and had way too much legal jargon in them. Landowners just didn't like them.
When my son and I and ask permission, I provide the landowner with an information sheet that has photos of us, my truck, license plate, and our contact information and hunting/trapping licenses. I actually had one landowner who said no call me back later to say he changed his mind based on the info sheet. Seems to work well.
